Description: The Administration on Children, Youth and Families
(ACYF), Administration for Children and Families (ACF) of the
Department of Health and Human Services (DHHS) is requesting Office of
Management and Budget (OMB) clearance for interview instruments to be
used in the Head Start Family and Child Experience Survey (FACES). This
study is being conducted under contracts with Abt Associates Inc.

(with The CDM Group, Inc. as their subcontractor (#105-96-1930)) to
collect descriptive information on Head Start families, and Westat,
Inc. (with Ellsworth Associates as their subcontractor (#105-96-1912))
to collect information on Head Start performance measures. The design
calls for three rounds of data collection. A nationally representative
group of 2,400 families with children enrolled in approximately 160
centers in 40 Head Start programs will be identified in Spring, 1997.
At that time, Head Start staff and parents will be interviewed,
classroom observations will be completed, and children will be
assessed. The second data collection period will occur in Fall, 1997.
Again, staff and parents will be interviewed, and children will be
assessed and observed in their classrooms. At that time children from
the Spring, 1997 sample that left Head Start to enter kindergarten
following the 1996-97 Head Start year will be replaced by a
representative sample of children just entering Head Start. All
families, including those whose children entered kindergarten in Fall,
1997 will be tracked through the school year. The final data collection
effort will occur in Spring, 1998 and involve all families and children
identified in the earlier two data collection periods.

A subgroup of 120 families will be identified from the Spring and
Fall, 1997 samples for participation in the Validation Substudy. The
Validation Substudy data collection will require home visits to
participating families at each major data collection point and a series
of monthly contacts between data collections periods. The monthly
contacts will begin with the Spring, 1997 data collection and continue
through December, 1998.
This schedule of data collection is necessitated by the mandates of
the Government Performance and Results Act (GPRA) of 1993 (Public Law
103-62), which requires that the Head Start Bureau move expeditiously
toward development and testing of Head Start Performance Measures, and
by the 1994 reauthorization of Head Start (Head Start Act, as amended,
May 18, 1994, Section 649 (d)), which requires assessment of Head
Start’s quality and effectiveness.

Description: The Administration on Children, Youth and Families
(ACYF), Administration for Children and Families (ACF) of the
Department of Health and Human Services (DHHS) is requesting comments
on plans to extend the Head Start Family and Child Experience Survey
(FACES). This study is being conducted under contract with Westat, Inc.
(with Elsworth Associates and the CDM Group as their subcontractors)
(#105-96-1912) to collect information on Head Start performance
measures. This revision is intended to extend the current design to a
national probability sample of 43 additional Head Start programs in
order to ascertain what progress has been made since 1997 in meeting
Head Start program performance goals.
FACES currently involves seven phases of data collection. The first
phase was a Spring 1997 Field test in which approximately 2400 parents
and children were studies in a nationally stratified random sample of
40 Head Start programs. The second and third phases occurred in Fall
1997 (Wave 1) and Spring 1998 (Wave 2) when data were collected on a
sample of 3200 children and families in the same 40 programs. Spring
1998 data collection included assessments of both Head Start children
completing kindergarten (kindergarten field test) as well as interviews
with their parents and ratings by their kindergarten teachers. In the
fourth and fifth phases, follow-up continued for a second program year,
plus a kindergarten follow-up. The sixth and seventh waves of data
collection involve data collection in spring of the first-grade year

for both cohorts of children, those competing kindergarten in spring
1999, and those completing kindergarten in spring 2000. The current
plan is to extend data collection to a new cohort of 2825 children and
families in a new sample of 43 Head Start programs.

on plans to amend the Head Start Family and Child Experiences Survey
(FACES) data collection. This study is being conducted under contract
with Westat, Inc. (with Ellsworth Associates and the CDM Group as their
subcontractors) (#105-96-1912) to collect information on Head Start
performance measures. The current revision is intended to include 8
additional sites participating in the Head Start Quality Research
Center Consortium. These Head Start program-university researcher
partnerships, funded under cooperative agreements with ACYF, will be
conducting evaluations of interventions designed to enhance the school
readiness of Head Start children in such areas as literacy and socialemotional development, through improvements in curriculum, training,
and assessment practices. This amendment will include use of
subsections of the FACES instrument battery to obtain information about
classroom quality and child performance both before and after the
implementation of the interventions. Data from local sites can then be
compared to the data available from the FACES national sample.

Start children, interviews with their parents, and ratings by their
Head Start teachers. Further, site visitors will interview Head Start
teachers and make observations of the types and quality of classroom
activities.
The second wave, which will be a repeat of the Fall 2003 data
collection, will occur in Spring 2004 when the sample children are at
the end of their first year of Head Start.
The third wave will occur in Spring 2005, and will involve followup with children who at this time are either completing a second year in
Head Start, or completing kindergarten. For those children who are
still attending Head Start, data collection will follow the same
procedures as in Spring 2004. For those children attending
kindergarten, data collection will include assessments of Head Start
children, an “update” survey of the information collected from the
parent interview, and ratings of the children's academic progress and
school adjustment by kindergarten teachers.
The fourth wave of data collection will occur in Spring 2006.
Children who attended kindergarten the previous year will not be
included in this wave. The procedures for this effort will be the same
as for kindergartners in Spring 2005.
For the Head Start Quality Research Centers, 100 children in eight
sites will be followed during each of two program years, 2003-2004 and
2004-2005. FACES procedures will be carried out, including child
assessments, parent and teacher interviews, and observations of types
and quality of classroom activities.

Data will be collected on a sample of approximately 2,800 children and families from about 350
classrooms across 60 Head Start programs. Data collection will include assessments of Head
Start children, interviews with their parents, and ratings by their Head Start teachers. Further,
site visitors will interview Head Start teachers and make observations of the types and quality of
classroom activities.
The second wave in spring 2007, will be very similar to the Fall 2006 data collection, except
that we will not repeat interviews with the Head Start staff interviewed in the fall, and will add
an interview of the family services coordinator. The children in the second wave will be at the

Description: The Administration for Children and Families (ACF) of the Department of Health
and Human Services (HHS) is requesting comments on plans to collect data on a new cohort for
the Head Start Family and Child Experiences Survey (FACES). ACF is conducting this study to
collect information on Head Start performance measures and has contracted with Mathematica
Policy Research, Inc. (with Juarez and Associates and Educational Testing Service as their
subcontractors) (contract #HHSP23320052905YC) for this purpose.
FACES will involve five waves of data collection. The first wave will occur in fall 2006.
Data will be collected on a sample of approximately 3,650 3- and 4-year-old children and their
families from about 350 classrooms across 60 Head Start programs. Data collection will include
assessments of Head Start children, interviews with their parents, and ratings by their Head Start
teachers. Furthermore, site visitors will interview Head Start teachers and management staff.
The second wave in spring 2007 will be very similar to the fall 2006 data collection, except
that we will not repeat interviews with the Head Start staff interviewed in the fall. The children
in the second wave will be at the end of their first year of Head Start. Trained staff will also do
observations of children’s Head Start classrooms.
The third wave will occur in fall 2007 and will involve follow-ups with children who at this
time are starting their second year in Head Start. Data collection will follow the same procedures
as in fall 2006.
The fourth wave will occur in spring 2008 and will involve follow-ups with children who at
this time are either completing a second year in Head Start or completing kindergarten. For those
children who are still attending Head Start, data collection will follow the same procedures as in
spring 2007. For those children attending kindergarten, data collection will include assessments
of children, an “update” survey of the information collected from the parent interview, and
ratings of the children’s academic progress and school adjustment by kindergarten teachers.
The fifth wave of data collection will occur in spring 2009. Children who attended
kindergarten the previous year will not be included in this wave. The procedures for this effort
will be the same as for kindergartners in spring 2008.

This schedule of data collection is necessitated by the mandates of the Government
Performance and Results Act (GPRA) of 1993 (Pub. L. 103-62), which requires that the Head
Start Bureau move expeditiously toward development and testing of Head Start Performance
Measures and, by the 1994 reauthorization of Head Start (Head Start Act, as amended, May 18,
1994, Section 649 (d)), which requires periodic assessments of Head Start’s quality and
effectiveness.

60 that are used to collect data on children’s school readiness outcomes to 180 for the purpose of
conducting observations in 720 Head Start classrooms. Program director, center director, and
teacher surveys will also be conducted in the spring. Plus features include additional survey
content of policy or programmatic interest, which may include more programs being sampled.
This notice is specific to the data collection activities needed to recruit Head Start programs and
centers into FACES 2014–2018. A future notice will provide information about data collection
for the Core and Plus studies.
A total of 230 Head Start programs and 460 Head Start centers will be selected to participate in
FACES 2014– 2018. The Core Study will include a nationally representative sample of 180
programs, with up to 50 additional programs potentially selected for Plus studies. For the Core,
the 60 programs participating in the Core child-level data collection will be contacted and
recruited for the study in spring 2014. In fall 2014, the remaining 120 programs will be
contacted. All 180 programs will be contacted a second time in fall 2016 to confirm their
continued participation in the Core spring 2017 data collection. The 50 Plus study programs
would be recruited at a similar time as the Core study programs (i.e., spring 2014 or fall
2014/2016) depending on the nature of the study being conducted.
The method of data collection for recruitment of all programs will include telephone
conversations with program directors and on-site coordinators who serve as liaisons between the
FACES study team and the Head Start centers. These calls will inform program staff about the
purpose of the study and will gather lists of centers in each program in order to compile the
center sampling frame.
The purpose of this data collection is to support the 2007 reauthorization of the Head Start

8461 02/12/2014). This notice describes the planned data collection activities for the FACES
Core study and Plus studies. Direct child assessments, parent surveys, and teacher child reports
for the Core study are included in this clearance package. Additionally, we describe instruments
to support the Core study at the program and classroom levels and the Plus studies anticipated for
future submission. Since these instruments will be informed by initial findings of FACES and
emerging policy needs, they cannot be fully specified at this time. However, we describe the
respondents and data collection methods, estimated respondent burden, and how the information
will be used to the extent possible at this time. Subsequently, when fully developed in a manner
consistent with the description provided in this 60-day notice and prior to use, we will submit
these materials for a 30-day public comment period under the Paperwork Reduction Act.
Methods for Core data collection start with site visits in fall 2014 to 60 Head Start programs to
directly assess the school readiness skills of 2,400 children sampled to participate in FACES.
Parents of sampled children will complete surveys on the Web or by telephone about their
children and family background. Head Start teachers will rate each sampled child (approximately
10 children per classroom) using the Web or paper-and pencil forms. These activities will occur
a second time in spring 2015. Additionally, the program sample size will increase to 180
programs in the spring to collect program- and classroom-level data. The methods of data
collection for this phase will feature site visitors conducting observations of the types and quality
of classroom activities. Head Start program directors, center directors, and teachers will
complete surveys about themselves and the services and instruction at Head Start. The programand classroom-level data collection will occur a second time in spring 2017.
Plus study data collection will parallel the Core design in many ways, including recruitment and
data collection procedures, to add new respondents, include new populations, or expand on the

information gathered in the Core study. Additional early care and education administrators or
providers (such as Education Coordinators or Family Service Staff) may be sampled. Plus studies
may involve data collection in additional programs, such as programs serving different
populations or programs implementing specific interventions. Data collection for these Plus
studies may include child assessments, parent surveys, teacher child reports, and staff surveys.
Plus studies may also feature topical modules to gather information in greater depth on particular
topics (for example, parent engagement or program functioning). The methods of data collection
will involve new methodologies such as qualitative interviews and supplemental surveys with
expanded content.

Methods for Core data collection start
with site visits to 120 centers in 60 Head
Start programs to sample classrooms
and children for participation in the
study. Field enrollment specialists (FES)
will request a list of all Head Startfunded classrooms from Head Start staff
and will ask for the teacher’s first and
last names, the session type (morning,
afternoon, full day, or home visitor), and
the number of Head Start children
enrolled. Then for each selected
classroom the FES will request the
names and dates of birth of each child
enrolled. Approximately two weeks
later, assessors will go to the 60 Head
Start programs to directly assess the
school readiness skills of 2,400 children
sampled to participate in FACES.
Parents of sampled children will
complete surveys on the Web or by
telephone about their children,
activities they engage in, and family
background. Head Start teachers will
rate each sampled child’s social and
emotional skills (approximately 10
children per classroom) using the Web
or paper-and-pencil forms.
The purpose of the Core data
collection is to support the 2007
reauthorization of the Head Start
program (Pub. L. 110–134), which calls
for periodic assessments of Head Start’s
quality and effectiveness. As additional
information collection activities are
fully developed, in a manner consistent
with the description provided in the 60day notice (79 FR 11445) and prior to
use, we will submit these materials for
a 30-day public comment period under
the Paperwork Reduction Act.

Title: Head Start Impact Study
Participants Beyond 8th Grade.

Description: The Administration for
Children and Families (ACF) within the
Department of Health and Human
Services (HHS) will collect follow-up
information from children and families
in the Head Start Impact Study. In
anticipation of conducting a future
follow-up for the study, ACF will collect
information necessary to identify
respondent’s current location and
follow-up with respondents in the
future.
The Head Start Impact Study is a
longitudinal study involving 4,667 firsttime enrolled three- and four-year-old
preschool children across 84 nationally
representative grantee/delegate agencies
(in communities where there were more
eligible children and families than can
be served by the program). Participants
were randomly assigned to either a
Head Start group (that could enroll in
Head Start services) or a control group

(that could not enroll in Head Start
services but could enroll in other
available services selected by their
parents). Data collection for the study
began in fall of 2002 and continued
through late spring 2008 to include the
participants’ 3rd grade year. Location
and contact information for participants
has been collected every spring
beginning in 2009 and continued
through spring 2014.
ACF will continue to collect a small
amount of information for the sample
through the spring of the participant’s
12th grade year. To maintain adequate
sample size, telephone interviews (with
in-person follow-up as necessary) will
be conducted in order to update the
children’s status and their location and
contact information. This information
will be collected from parents or
guardians in the spring of 2015 and
2016. Updates will take about 20
minutes to complete.
Respondents: The original sample of
4,667 treatment and control group
members in the Head Start Impact
Study, less 432 families that have given
a ‘‘hard’’ refusal to participate in the
study (e.g., adamantly refused to
participate or threatened interviewers if
they were contacted again). The number
of respondents for this requested data
collection is 4,235.

Development (NICHD), National
Institutes of Health (NIH), grants 2 R01
HD021896 and 5 R01 HD042201–05 and
National Center for Research Resources
(NCRR), NIH, grant 5 R01 RR013438–07.
ORI found that the Respondent
intentionally, knowingly, and recklessly
fabricated and falsified data reported in
the following published paper:
• Deb, K., Sivarguru, M., Yong, H., &
Roberts, R.M. ‘‘Cdx2 gene expression
and trophectoderm lineage specification
in mouse embryos.’’ Science 311:992–
996, 2006 (hereafter referred to as
‘‘Science 311’’); this paper was retracted
on July 27, 2007
An earlier version of Science 311 had
been previously submitted to Nature on
or about June 24, 2005 (hereafter
referred to as ‘‘Nature #1’’). It was
revised and resubmitted to Nature on or
about August 24, 2005, and ultimately
was rejected by Nature on September
14, 2005 (hereafter referred to as
‘‘Nature #2’’).
Specifically, ORI finds by a
preponderance of the evidence that the
Respondent engaged in misconduct in
science by intentionally, knowingly,
and recklessly:
1. Falsifying and/or fabricating three
panels of data in Figure 1 (Figures 1C,
1D, and 1E) in Science 311 and in

Nature #1 and Nature #2, by photomanipulating confocal fluorescent
images to falsely represent three-,

cells derived from a leading blastomere
would be Cdx2 negative
2. using photo-manipulation to falsify
and fabricate at least 13 panels of
confocal image data in Figures 2, 3, and
S2, including Figures 2K, 2L, 2Q, 2R,
2V, 2X, 3G, 3H, 3I, S2s, S2t, S2u, and
2W, in Science 311 and in
corresponding figures in Nature #1 and
Nature #2 so that these images falsely
supported the central premise in
Science 311 that Cdx2-expressing cells
were peripherally located in the embryo
3. falsifying Figures 2G, 3J, 3L, S2V,
S2X, S6I, S6J, and S6K in Science 311,
Figures 2A, 2C, S4v, and S4x in Nature
#1, and Figures 2G, 3I, 3J, and 3K in
Nature #2 by reusing and re-labelling
the same image to represent different
embryos and different experimental
conditions
4. falsifying Figure 4 in Science 311
and corresponding figures submitted in
Nature #1 and Nature #2 to falsely
illustrate that the first dividing cell of a
two-cell mouse embryo will ultimately
differentiate into the trophoblast;
specifically, Respondent:
• Falsely colored and
photomanipulated a single bright-phase
image of a three-cell embryo to make it
appear as four separate embryos that
had been differentially injected with
TRD
• falsely colored and
photomanipulated a four-cell embryo to
make TRD appear distinctly located in
the lagging cell and in its descendent
cell, when the actual embryo contained
diffuse staining within the sub-zonal,
extracellular space
• photomanipulated a damaged, nonviable two-cell embryo to make it
appear viable
• re-used, falsely colored, and
relabeled seven images from an
unrelated experiment to falsely
represent a time lapse course of eight
different images
5. falsifying Figures 5K, 5L, 5N, and
5O in Science 311 by photomanipulating a single confocal image to
falsely represent four different images at
two different stages of embryonic
development. The images also were
presented as Figures 4k, 4l, 4n, and 4o
in Nature #1.

school readiness skills of Head Start
children, survey their parents, and ask
their Head Start teachers to rate
children’s social and emotional skills. In
addition, FACES will include
observations in Head Start classrooms,
and program director, center director,
and teacher surveys. FACES Plus
studies include additional survey
content of policy or programmatic
interest, and may include additional
programs or respondents beyond those
participating in the Core FACES study.
Previous notices provided the
opportunity for public comment on the
proposed Head Start program
recruitment and center selection process
(FR V. 78, pg. 75569 12/12/2013; FR V.
79, pg. 8461 02/12/2014) and the childlevel data collection (FR V. 79, pg.
11445 02/28/2014; FR V. 79; pg. 27620
5/14/2014). This 30-day notice describes
the planned data collection activities for
spring 2015. Classroom sampling
information collection, parent
supplement survey content, and teacher,
program director, and center director
surveys for the Core study are included
in this information collection request.
Additionally, parent and staff

interviews are included for FACES Plus
studies.
Methods for Core data collection
involve returning to 60 programs visited
in fall 2014, where we will conduct
previously approved activities of child
assessments, parent surveys, and
Teacher Child Reports. An additional
240 Head Start centers in 120 Head Start
programs will be visited to sample
classrooms. Field enrollment specialists
(FES) will request a list of all Head
Start-funded classrooms from Head Start
staff. Across all 180 programs, 720
teachers, 180 program directors, and 360
center directors will complete surveys
about the Head Start classroom or
program and their own background
using the Web or paper-and-pencil
forms.
Two Plus studies are also planned in
spring 2015. First, a topical module on
family engagement will be conducted in
the 60 programs participating in childlevel data collection. All parents will
complete a 5-minute spring supplement
to the parent survey about parent-staff
relationship and communication and
community support. All teachers will
complete a 5-minute Plus survey about
parent-staff relationships (FPTRQ) as

part of the core teacher survey. A
subsample of Head Start parents (n=360)
and Family Service Staff (n=180) will be
interviewed on parent involvement in
Head Start and program outreach and
engagement practices. Interviews will
take about one-hour and will be
conducted by phone using paper-andpencil guides. Second, within the 120
programs participating in classroomonly-level data collection, 480
classroom teachers will complete a new
measure of program functioning (5EEarly Ed) to examine its reliability and
validity for future FACES use.
The purpose of the Core data
collection is to support the 2007
reauthorization of the Head Start
program (P.L. 110–134), which calls for
periodic assessments of Head Start’s
quality and effectiveness. As additional
information collection activities are
fully developed, in a manner consistent
with the description provided in the 60day notice (79 FR 11445) and prior to
use, we will submit these materials for
a 30-day public comment period under
the Paperwork Reduction Act.

AI/AN FACES fall 2015 data
collection includes site visits to 37
centers in 22 Head Start programs to
sample classrooms and children for
participation in the study. Field
enrollment specialists (FES) will request

will ask for the teacher’s first and last
names, the classroom session type
(morning, afternoon, full day, or home
visitor), and the number of Head Start
children enrolled in each classroom.

Works (NEW) program plan is the
application for NEW program funding.
As approved by the Department of
Health and Human Services (HHS), it
documents how the grantee will carry
out its NEW program. The NEW
program plan guidance provides
instructions for preparing a NEW
program plan and explains the process
for plan submission every third year.
There are two versions of this plan
guidance: One for tribes that include
their NEW program in a Public Law
102–477 project, and one for tribes that
do not. The primary difference between
the guidance documents is in the
instructions for how to submit the plan.
The NEW program report provides
information on the activities and
accomplishments of grantees’ NEW
programs. The NEW program report and
instructions specify the program data
that NEW grantees report annually.
