Abstract: NRC regulation require NRC licensees to report incidents and events involving the use of radioactive by-product material, and saource material, such as thos involving a radiation over- exposures, leaking or contaminated sealed source(s), release of excessive contimination of radioactive material, lost or stolen radioactive material, equipment failures, and abandoned well logging sources. NRC is requesting that the Agreement States provide information on the initial notification, response actions, and follow-up investions on all events involving the use of nuclear materials regulated pursuant to the AEA.
