Abstract: NRC policy requires licensees to report incidents and events involving the use of radioactive byproduct, source materal, such as those involvinag a radiation overexposure, leaking, or contaminated sealed sources, lost or stolen radioactive material, equipment failures, and abandoned well logging sources and medi- cal misadministrations. NRC is requesting that the Agreement States provide information on the initial notification, response actions, and follow-up investigations on events and medical misadministration involving the use of the nuclear materials.
