Terms of Clearance: FCC has permission to revise the FCC From 457. FCC should ensure that carriers below the de minimis threshold are notified that they must complete the FCC 47 but only retain it in their records. The FCC shall also direct the Adminstrator or Temporary Administrator of the Fund to adopt and implement electronic filing for all required submissions related to the USF program.

Abstract: The Commission revised the FCC form 457 to update the form and instructions consistent with the 4th Order on Reconsideration to provide a list of entities that are excluded from Federal universal service contribution requirements. The revised work sheet reflects, among other things, the new deminimis exemption adopted in the 4th Order on Recon. The instructions have been revised to clarify, consistent with the 2nd Order on Recon., that quarterly contributions will be paid in monthly installments. FCC form 457 must be filed Sept. 1 and March 31 each year and is used to administer the USF program.
