Abstract: FORM IS USED TO DETERMINE A PERSON WHO INTENDS TO MANUFACTURE LIQUOR BOTTLES AND ASSIGN A NUMBER OF PLANTS MAKING LIQUOR BOTTLES. DESCRIBES THE PERSON WHO INTENDS TO MANUFACTURE, THE LOCATION OF THE PLANT, THE USE OF MATERIALS TO MAKE LIQUOR BOTTLES, AND THE NUMBER ASSIGNED TO PLANTS MAKING LIQUOR BOTTLES. THE NUMBER OF THE PLANTS IS PLACED ON THE LIQUOR BOTTLE IN ITS MANUFACTURE.
