The National HIV Behavioral Surveillance system (NHBS) is a supplemental surveillance project designed to describe the HIV prevalence and behaviors related to HIV acquisition and prevention among the three populations at highest risk for HIV in the United States: men who have sex with men, persons who inject drugs, and heterosexually active persons at increased risk for HIV infection. For this Revision, interview data collection instruments were revised and the number of health departments participating in the NHBS System will decrease, in addition to other minor changes.
The NHBS interview data collection instruments were revised to reduce respondent burden and enhance data quality, resulting in a decrease in annualized burden hours from 6,600 to 3,398 and an increase in participating health departments to 21. Key changes included updating race/ethnicity questions to meet new standards, removing or modifying questions related to HIV prevention and substance use, and eliminating repetitive or low-priority items. These adjustments improve the clarity, efficiency, and relevance of the assessments without altering the overall scope or time required for participants.

Key changes involve expanding questions on race/ethnicity to align with new data standards, updating behavioral assessment items to reflect emerging HIV prevention strategies such as PrEP and Doxy PEP, and addressing fentanyl use. Numerous questions were deleted to eliminate redundancy and low-priority content, resulting in a decrease in total annualized burden hours from 6,600 to 3,398.The number of participating health departments will increase to 21, while project activities and core methods remain unchanged from previous cycles.
