Abstract: This form is used by school officials and employers to certify enrollment information to the Department of Veterans Affairs (VA). VA uses the information to determine the correct rate of payment and the periods of payment to persons eligible for education benefits under chapters 30, 32, 33, and 35 of title 38, U.S.C., chapter 1606 and 1607 of title 10 U.S.C., and Sections 901 and 903 of Public Law 96-342.

Short Statement: This information collection renewal resulted in a decrease to burden hours due to a significant decrease in the number of responses received for the period(s) 2021, 2022 and 2023, and due to the use of three years of data, as opposed to four years used for the previous renewal. This submission is therefore being submitted as a Revision. This information collections considers various types of enrollments such as paper hardcopies, electronic originals, and supplemental enrollments.
