QECP PHASE 3 APPLICATION REQUIREMENTS LIST
Purpose: This document will help facilitate entities through the Phase 3 application by providing a brief
overview of each application component.
Element 3.1: Standard Measure Use Required Documentation

Complete the QECP Measure Information Workbook (template provided by QECP team, located
in the QECP Phase 3 Toolkit) demonstrating compliance with the program requirements for any
standard measures selected for public reporting.
Rationale for selecting the standard measure, information on the relationship to existing
measurement efforts, and relevance to the population examined.
Attestation (yes/no) whether the QE is following the standard measure specifications and
guidance for risk adjustment, outliers, and attribution.

Element 3.2: Alternative Measure Use Required Documentation

Complete the QECP Measure Information Workbook (template provided by QECPteam, located
in the QECP Phase 3 Toolkit) demonstrating compliance with the program requirements for any
alternative measures selected for public reporting.
Evidence that the proposed alternative measures are more valid, reliable, responsive to
consumer preferences, cost- effective, or relevant to dimensions ofquality and resource use
measures not addressed by standard measures.
Documentation of stakeholder engagement and support for the use of thealternative
measure(s).

Element 3.3: Provider and Public Report Design Required Documentation

Prototypes of the confidential provider performance report and public performance report
containing an understandable description of the measure usedto evaluate the performance of
providers or any provider rating approaches
Description of the process that will be used for making QE reports available for thepublic.
