Abstract: This is a New collection request for a new OMB Control Number. The information collections will provide DASH with data to generate internal reports that will identify successful and problematic surveillance and programmatic areas. In addition, both information collections will allow DASH to determine if recipient agencies are completing the required activities of the NOFO on time, as well as identifying problems in implementation.
