Abstract: The Department is updating the Privacy Act Statement on these attestations to identify the change in the Systems of Records Notice (SORN) from the Common Services for Borrowers SORN (11-16-16) to the Customer Engagement Management System (CEMS) SORN (18-11-11) which replaces the “Office of the Student Loan Ombudsman Records”. This change provides updated uses and expands the information held under the revised SORN to users of the form. Additionally, there is a change to the request for an applicant’s social security number. The form now will only request the last four digits for improved security.
