The Crime Control Act of 1990 (42 U.S.C. 13041) requires individuals who are providing child care services to self-report any arrests, charges, or convictions of specific offenses. Individuals are also required to indicate if their actions have met the Family Advocacy criteria for child maltreatment. Reporting is voluntary, but failure to furnish required information may result in an unfavorable adjudication decision and may affect suitability of working with or around children.

This is a reinstatement, with change, of a previously approved collection for which approval has expired. The burden has increased since the previous approval due to the use of an average wage of $26.00 per hour rather than using the minimum wage to calculate the burden. Additionally, this submission accurately reflects the amount of time needed to respond using the DD Form 2981, which was underestimated in the last submission. There are no increases in the number of responses.
The percentage of electronic submissions is decreasing to accurately reflect the use of the DD Form 2981, which has no electronic capabilities. The Department will be working to add these capabilities over the coming approval period, but currently the form is only accepted in hard copy.
