Good evening Mr. Silverblatt,
Thank you for your comments on the HUD-92403-CA form which was published in the Federal
Register. The form now includes the boxes for the Tax ID Number and the Owner’s DUNS. Also we
can’t revise the form to show the items requested by funding source. Also the shading in Column B
can’t be eliminated.
