Justification for Non-Substantive Changes to the Collection
Change 1: We are adding the beneficiary’s Social Security Number on every
page of Form SSA-3033.
Justification: Currently only the cover sheet of Form SSA-3033 displays the beneficiary’s SSN. If any subsequent pages are separated from the cover sheet the technician will not be able to identify who the subsequent pages belong to.
