In its March 2012 supporting statement which was submitted and approved by OMB, the Commission implemented some of the information collections required by this Order, FCC 11-161, but also noted that at a later date it planned to submit additional revisions or new requirements for OMB to review and approval to address other reforms adopted in the Order. This current revision addresses those additional requirements. See the uploaded supporting statement for the specific requirements.
Note: There are no changes to FCC Form 525 which is part of this information collection. However, the Commission is requesting OMB approval for a new FCC Form 481, Annual Reporting Form for High-Cost Recipients and other new and modified information collection requirements.

The estimated total annual burden hours have increased for all requirements now under this OMB control number. The total hourly burden reported previously in 2012 was estimated at 163,435 hours and, as a result of the changes to the requirements in this IC in items 12h through 12q, have now increased the total annual burden to 272,017 hours. The increase is due to the expanded goals of Connect America and the implementation of GAO recommendations regarding the oversight of the universal service program.
