Approximately 18 (or 38) months ago, during a visit to your clinic for routine screening, you agreed to participate in the CDC Cx3 Study.

During that visit you completed a survey while you were at the clinic.

In about a week we will be mailing you the first (or second) of two follow-up

surveys that we would like you to fill out.



Please take a few minutes to fill out and return the survey when you receive it in the mail. Your answers will give CDC valuable information to help them to provide cancer screening to low income, uninsured women.
