Terms of Clearance: This collection is cleared for one year with the understanding DOD will address the issues provided in the April 1, 2010, passback from OMB. Namely, to provide an understanding of why a PIA or SORN is not needed for this system. Also, the purpose statement should be re-written to be clearer for the individual filling out the form. A new 60 day notice is not required with re-submittal.

Abstract: Under Title 10 U.S. Code section 1552 allows current and fomer Service members to apply to their respective Board for the Correction of Military Records (BCMR) to correct errors of injustices in their military records. The BCMRs of the Service are the highest administrative board and appellate review authorities for resolving military personnel disputes. The DD Form 149 "Application for Correction of Military Record Under the Provisions of Title 10, U.S. Code, Section 1552," will be submitted by current or former Service members to request errors or injustices in their military records be corrected.
