You have been identified as someone who has received care within the MOVE! Weight 
Management Program at one or more VA healthcare facilities. The Veterans Health 
Administration (VHA) wants to make the VA your first choice for all your health care needs, and 
we need your help. The VHA has partnered with Synovate, a well‐known survey firm, 
to conduct an important research study about your experiences with the MOVE! Program.  
 
Within two weeks you will be getting a survey about your experiences with the MOVE! Program 
at the VA.  This survey is voluntary. You are not required to respond to it and your answers will 
not affect your eligibility for VA benefits.  However, we do hope you will choose to complete 
the survey because the answers that you give will help us improve the MOVE! Program.
