JUSTIFICATION

The ICR was approved by OMB on 01/09/2009, with changes.

A minor error in the approved title has been noted. The approved title does not include “Screening” as intended.

CDC requests correction of the title to ensure consistency with the Supporting Statement and published Federal Register Notices.
