Abstract: CDC is seeking OMB approval to reinstate data collection for the Resources and Services Database for the CDC National Prevention Information Network (formerly known as the National AIDS Clearinghouse). The database contains entries on approximately 19,000 organizations and is the most comprehensive listing of HIV/AIDS, STD, and TB resources and services available throughout the US. This request is for a 3 year approval.

Short Statement: All modifications made to the instruments reflect the mission of NCHSTP and are in line with the previous data collection approval. The few questions added to the instruments are related to the data collection subject matter and are consistent with NCHSTP mission. Authorization for use of the revised Questionnaire, telephone script of the Questionnaire, and requests for annual telephone update or email update is requested for a three-year period.
