My explanation for the changes follows:

After anecdotal discussion with some of the survey respondents, we understood that the wording of the questionnaire, and the order of possible choices, could be simplified without altering the burden to the recipient or the number of people targeted for the survey this calendar year. I have highlighted the revised survey to reflect content changes and, in some cases, the ordering of the responses. Please note that question 4 from the current survey was omitted entirely in the revised survey to elicit clearer responses.
