Abstract: The new form will be used by Organizational Representative Payees when applying for permission to collect a fee for their payee services. The information collected will be used to determine if the applicant meets the fee for service requirements and provides all required documentation as established by Section 205(j)(4)(A)(i) of the Social Security Act.
