Terms of Clearance: This package combines the generic customer satisfaction surveys for ATSDR under a CDC OMB number. This arrangement is approved for 1 year on a trial basis. At the end of that year, OMB will assess any problems with having the ATSDR satisfaction surveys under a different OMB code than other ATSDR paperworks.
