Terms of Clearance: Approved. Total and Permanent Disability discharges will be revised to comply with new regulatory requirements that take effect on July 1, 2002. ED will submit a revised package in advance of the July 1 effective date, reflecting the changes in that discharge form. At that time, ED will provide responses to the comments recieved at this time and also in response to the new Federal Register notice that was published regarding the revised form. Additionally, the license number of the physician (required in the disability discharge form) should be checked and tracked by ED. Upon resubmission of the disability discharge form, ED shall provide additional detail of the process for verifying and tracking physician license numbers. The full package is being cleared for 6 months in order to allow for time to clear the new disability discharge forms. Upon resubmission, the full package will be cleared again.

Abstract: These forms will serve as the means of collecting the information necessary to determine whether a FFEL or Direct Loan borrower qualifies for a loan discharge based on total and permanent disability, school closure, false certification of student eligibility, or unauthorized signature. The school closure discharge application may also be used by Perkins Loan borrowers applying for a closed school discharge.
