Additionally, the FCC shall submit, as soon as practicable, a change worksheet that demonstrates actual burden reduction as claimed in the ICR. The FCC's_request to not print the expiration date on the forms is granted, on the condition that the FCC solicit public comment on this particular issue when it asks for public comment on its resubmittal.

Pursuant to the Communications Act of 1934, as amended, telecommunications carriers (and certain other providers of telecommunications services) must contribute to the support and cost recovery mechanisms for telecommunications relay services, numbering administration, number portability, and universal service. The Commission has combined the reporting requirements associated with these mechanisms so that carriers need only file one worksheet to satisfy all of these requirements. The information will be used to calculate carriers' contributions.
