Terms of Clearance: This collection is approved on the condition that FDA update form 2767 by revising the burden statement to comply with the Paperwork Reduction Act of 1995 and by revising the expiration date. OMB notes that FDA allowed approval of this collection to lapse which is a violation of the Paperwork Reduction Act. FDA shall take additional steps to ensure that such lapses do not occur in the future.
