Terms of Clearance: The FCC has permission to delete the OMB expiration date for this form provided an edition date is added. The FCC shall take steps to allow electronic filing of this information and should consider making this form available on the internet for direct filing. Finally, the FCC must include the language required by the Debt Collection Improvement Act (P.L. 104-134) Section 31001 specifically informing the filer that the FCC intends to use the TIN for the purposes of collecting and reporting on any delinquent amounts arising out of that person's relationship with the Government. The FCC shall ensure the TIN is redacted prior to placing these forms on public display.
