Terms of Clearance: Approved for use through 3/97 under the conditions that: 1) NCHS amends the survey instrument to incorporate the new disclosure statements pursuant to the Paperwork Reduction Act of 1995. For the public docket, NCHS must submit to OMB the revised survey instrument and disclosure statement; and 2) the next submission for OMB review includes a comparison of the NHHCS (in particular the current and discharged patient questionnaires) with the home health section of MEPS, and an analysis of further opportunities for coordinating between these two survey efforts.
