Abstract: The information collections are conducted in support of disability programs. The information collected from a consultative exam (CE) is used to make disability determinations when the claimant's own medical sources cannot or will not provide the information. The information collected from potential CE providers is used to verify the provider's credentials and licenses. The medical evidence of record (MER) information is used to determine a person's physical and/or mental status prior to making a disability determination.
