Terms of Clearance: This collection is approved as amended by ACF's memorandum of 6/2/95. However, this collection has only been given a one year clearance. ACF must revise this form and the Annual Report on Households Assisted by LIHEAP (OMB #0970-0060) to reflect the new requirements included in the 1994 reauthorization of the LIHEAP statute, including a requirement that LIHEAP develop model performance goals and measures. The draft Annual Report on Households which was submitted with this package has not been approved by OMB and should not be sent out to States to collect the information. The Annual Report should be amended prior to its expiration at the end of 12/31/95.
