MORTGAGEES APPLY FOR APPROVA TO PARTICIPATE USING FORM HUD-54112 AND CERTIFY THAT THE MORTGAGE COMPLIES WITH STATUTORY AND REGULATORY REQUIREMENTS USING FORM HUD-54113.
