Abstract: THE FORM IS USED BY HUD TO MONITOR PHA PROGRESS IN IMPLEMENTING THE MODERATE REHABILITATION PROGRAM AND AS A MEANS OF APPROVING PHA REQUISITIONS FOR FUNDS. ALSO, THE FORM WILL ASSIST HUD IN IDENTIFYING THOSE PROJECTS WHERE A REDUCTION IN THE NUMBER OF UNITS UNDER AN ACCOU IS REQUIRED DUE TO UNDERUTILIZATION BY THE PHA.
