Abstract: THE INFORMATION COLLECTED BY THIS FORM WILL BE USED BY THE SOCIAL SECURITY ADMINISTRATION AS A BASIS TO PROVIDE ESTIMATES OF FUTURE SUPPLEMENTAL SECURITY INCOME (SSI) PAYMENTS FOR RECIPIENTS WHO ARE ABOUT TO RECEIVE A MONETARY REIMBURSEMENT FOR WORK PERFORMED.
