Abstract: OWNERS/OPERATORS MUST NOTIFY EPA OF CONSTRUCTION, MODIFICATIONS, STARTUPS, SHUTDOWNS, MALFUNCTIONS, DATE AND RESULTS OF PERFORMANCE TEST. OWNERS/OPERATORS MUST RECORD WHEN EACH EQUIPMENT LEAK IS DETECTED, THE DATE EACH REPAIR WAS MADE, AND WHETHER OR NOT LEAKS WERE REDUCED BELOW 10,000 PPM AFTER EACH REPAIR ATTEMPT. SEMI-ANNUAL REPORTS OF EQUIPMENTS LEAKS MUST BE MADE.
