Abstract: FORM 6458 IS USED TO MAKE ELECTIONS AND CERTIFICATIONS UNDER THE CRUDE OIL WINDFALL PROFIT TAX ACT OF 1980. THE FORM IS USED BY PERSONS SEEKING TO HAVE AN EXEMPTION FORM, OR A SMALLER AMOUNT OF TAX APPLY TO THEM. IT IS ALSO USED BY PERSONS WHO DESIRE TO WITHHOLD THE TAX FROM THE PRODUCERS. THE FORM IS ALSO USED BY THE PERSONS LISTED ABOVE TO REVOKE THESE ELECTIONS.
