Terms of Clearance: This information collection request is approved with the understanding that CDC plans to revise the response categories for 12 month recall of risk activities to include more quantitative parenthetical descriptors. CDC also plans to discuss revisions in the item count questionnaires with the contractor that would minimize any disincentiv caused by unnecessary emphasis on the critical response items.
