Abstract: THE INFORMATION REQUESTED WILL ASSIST THE DEPARTMENT IN SELECTING APPLICANTS WHICH MEET PROGRAM REQUIREMENTS AND DEMONSTRATE THE GREATES NEED FOR THE MODERATE REHABILITATION SINGLE-ROOM OCCUPANCY (SRO) PROGRAM FUNDS.
