Approved with the understanding that: 1) when form 4070A, Employee's Daily Record of Tips, is reprinted, it will contain the following statement in a prominent location, "you must keep a daily record containing the amount of cash tips and charge tips received directly from customers or from other employees, the amount of tips, if any, paid out to other employees through tip sharing, tip pooling or other arrangements, and the names of such employees. However, you do not need to use form 4070A to keep your daily record, and 2) Department and IRS staff will meet with OMB the week of April 10, 1989 to discuss the practical utility of the information contained on form 4070A and the utility of the information required to be reported by employers. The expiration date does not need to be printed on this form. Also, you may continue to use previous versions of this form until current supplies are exhausted. The OMB number should be added to the Paperwork Reduction Act notice.

EMPLOYEES RECEIVING AT LEAST $20 A MONTH IN TIPS MUST REPORT THE TIPS TO THEIR EMPLOYERS MONTHLY FOR PURPOSES OF PERMITTING EMPLOYMENT TAXES TO BE WITHHELD. FORMS 4070 AND 4070PR (PUERTO RICO ONLY) ARE USED FOR THIS PURPOSE. EMPLOYEES MUST MAINTAIN A DAILY RECORD OF TIPS RECEIVED. FORMS 4070-A AND 4070-A-PR (PUERTO RICO ONLY) ARE USED FOR THIS PURPOSE.
