Abstract: THE FORM PROVIDES A UNIFORM METHOD FOR RECORDING INFORMATION OBTAINED DURING AN INITIAL SURVEY OF A CONTRACTOR FACILITY. THE PURPOSE OF THE SURVEY IS TO (1) DETERMINE ELGIBILITY OF A CONTRACTOR FOR ENTRY AND PARTICIPATIO IN THE INDUSTRIAL SECURITY PROGRAM AND (2) DETERMINE ABILITY OF A FACILITY TO PHYSICALLY SAFEGUARD CLASSIFIED INFORMATION.
