Abstract: THIS FORM REQUESTS INFORMATION WHICH WILL BE USED TO EVALUATE THE SUITABILITY OF A PROGRAM OF EDUCATION. THE FORM ALSO REQUESTS INFORMATION CONCERNING PREVIOUS UNSATISFACTORY PROGRESS IN TRAINING. THE INFORMATION WHICH IS OBTAINED WILL BE USED TO MAKE DETERMINATIONS REQUIRED BY 38 U.S.C. 1674, 1724 AND 1791 PRIOR TO AUTHORIZATION OF BENEFIT PAYMENTS.
