Abstract: THIS FORM IS USED BY VETERANS TO REINSTATE THEIR GOVERNMENT LIFE INSURANCE AND/OR TOTAL DISABILITY INCOME PROVISION. THE DATA COLLECTE IS USED BY THE VA TO DETERMINE ELIGIBILITY FOR THE PURPOSE OF REINSTATEMENT. THE INFORMATION ON THE FORM IS REQUIRED BY LAW, 38 CFR SECTIONS 6.79 AND 8.23.
