Abstract: THE FORM IS USED BY INSUREDS TO APPLY FOR THE CHANGE OF ONE PERMANENT PLAN POLICY TO ONE HAVING A HIGHER RESERVE VALUE. THE INFORMATION ON THE FORM IS AUTHORIZED BY TITLE 38 U.S.C. SECTIONS 704 AND 742. THESE SECTIONS OF THE LAW REQUIRE THAT AN APPLICATION BE MADE BY THE INSURED PRIOR TO MAKING THE CHANGE.
