SOURCE MUST ALSO NOTIFY EPA OF CONSTRUCTION/RECONSTRUCTION/MODIFICATIO ANTICIPATED/ACTUAL START-UP DATE, AND DATE AND RESULTS OF THE PERFORMANCE TEST. RECORDS OF THE PERFORMANCE TEST RESULTS, MONITORED PRSSURE DROPS, AND START-UPS, SHUTDOWNS, AND MALFUNCTIONS MUST BE MAINTAINED TO ENSURE COMPLIANCE WITH THE STANDARD.
