Abstract: MORTGAGEES USE THIS FORM TO NOTIFY HUD THAT A PROJECT OWNER HAS DEFAULTED AND THAT AN ASSIGNMENT OR ACQUISITION WILL RESULT IF HUD AND THE MORTGAGOR DO NOT DEVELOP A PLAN FOR REINSTATING THE LOAN. THE NOTICE TRIGGERS HUD NEGOTIATION WITH THE MORTGAGOR.
