Abstract: THE INFORMATION OBTAINED BY THIS FORM IS NEEDED FROM A WORKER OR EMPLOYER IN ORDER TO OBTAIN A CERTIFICATE OF COVERAGE FROM THE U.S. SOCIAL SECURITY SYSTEM WHILE WORKING IN A FOREIGN COUNTRY. THIS CERTIFICATE WILL ENABLE THE WORKER TO BE EXEMPT FROM PAYING TAXES INTO A FOREIGN SOCIAL SECURITY SYSTEM.
