IDENTIFIES THE USER AND HIS LOCATION, WHETHER SPECIALLY DENATURED SPIRITS OR TAX-FREE ALCOHOL IS WANTED, THE PERSON FROM WHOM ARTICLES ARE TO BE PROCURED AND THE MAXIMUM ALLOWABLE TO BE SHIPPED AT ONE TIME DURING ONE YEAR. PRESHIPPER IDENTIFIES EACH SHIPMENT ON THE FORM.
