Abstract: FORM IS USED TO DETERMINE WHETHER A PERSON IS ELIGIBLE TO RECEIVE DENATURED SPIRITS OR ALCOHOL FREE OF TAX FOR CERTAIN OPERATIONS AND LIMITED AMOUNTS. DESCRIBES THE PERSON, BUSINESS, INTENDED USE OF DENATURED SPIRITS OR ALCOHOL AND AMOUNT THEREOF NEEDED AND INTENDED SUPPLIER. FORM, WHEN APPROVED, SERVES AS AN AUTHORITY FOR THE SHIPPER TO SEND THE PERSON DENATURED SPIRITS OR ALCOHOL FREE OF TAX.
