Abstract: FORM IS USED TO SUPPORT A DISTILLED SPIRITS PLANTS BOTTLING OPERATIONAL LOSSES OF TAX DETERMINED SPIRITS RECTIFIED AND/OR BOTTLED. FORM IDENTIFIES THE DISTILLED SPIRITS PLANT TRANSACTIONS ABOUT SPIRITS ENTERED INTO THE BOTTLING PREMISES BY QUANTITY AND SOURCE, AND OTHER TRANSACTIONS TO DETERMINE AMOUNT TO BE CLAIMED. FOR IS USED BY ATF TO VERIFY THE ACCURACY OF THE CLAIM FOR TAXES.
