DESCRIBES THE SHIPPER, PERSON TO WHOM SHIPPED, DETAILS ABOUT THE SHIPMENT AND FRUIT-FLAVOR CONCENTRATE FOR TAX LIABILITY PURPOSES AND RECEIPT BY THE CONSIGNEE.
