Abstract: THIS FORM DETERMINES WHETHER A CERTIFICATION FROM A STATE OF MULTI-STATE AUTHORITY IS NECESSARY FOR DISCHARGE INTO NAVIGABLE WATERS OF THE U.S. THE FORM REQUIRES A LICENSE OR PERMIT APPLICANT TO STATE THE LOCATION OF EXISTING OR PROPOSED ACTIVITIES CONNECTED WITH AN ATF ISSUED LICENSE OR PERMIT AND A DESCRIPTION OF THE TYPE, QUANTITY, AND POINT OF DISCHARGE OF ANY AFFLUENT INTO NAVIGABLE WATER OF THE U.S. FROM THE PROPOSED ACTIVITY.
