Abstract: NEED: TO ENSURE PROPER PAYMENT FOR THE MEDICARE/MEDICAID HOSPICE DEMONSTRATION FOR SERVICES PROVIDED TO THE BENEFICIARIES BY THE 26 HOSPICE ORGANIZATIONS SELECTED TO PARTICIPATE. USES: THE BILLING FORM WILL BE USED FOR SERVICES COVERED UNDER THIS DEMONSTRATION ON THE BASI OF REASONABLE COST SUBJECT TO RETROSPECTIVE COST REIMBURSEMENT.
